Payment for services may be made in a number of ways. Our primary source of payment is through the patients' insurance coverage. It is important for each patient to understand the terms of their coverage. Familiarizing yourself with your coverage will help you better understand what is required of you for each visit and how each bill is processed.
Please bring your insurance card to every visit. We will scan and store an image of your card in our billing system. However insurance plans change frequently so we ask to see your card to verify that we have the current image on file. If you have more than one insurance company, please bring all associated cards.
Co-pays are a contractual agreement between you and your insurance plan and will be due at the time of service. Co-pay amounts may be different for a specialist visit than they are for a primary care visit. Payment of co-insurance and deductibles is required for any balance due after claims have been submitted and processed by your insurance company. If you have questions or are unsure of your payment responsibilities, please call the phone number on your insurance card for customer assistance.
It is your responsibility to be aware if your health insurance requires a referral to a specialist. If so, please ensure your primary care physician processes a referral before making an appointment in order to receive the full benefits allowed by your insurance plan. All HMO plans require a referral from your primary care physician.
Prior to scheduling an appointment for patients with Workers Comp insurance, we require the following information:
If you are covered by Medicare, you will also be required to bring your Medicare card with you for verification and eligibility purposes. If you have supplemental coverage, please bring associated cards as well.
We do not bill automobile insurance companies. We will submit your claim to your health insurance as your primary insurance, except for Medicare, pursuant to New Hampshire state law. If you do not have health insurance, you will be considered Self-Pay.
Payment will be due at the time of service. A 10% discount will be granted if paid in full on the day of your appointment.
